Good
Faith
Estimate

Under the law as of January 1, 2022, health professionals must provide clients an estimate for medical items and services to those who do not have insurance or will not utilize insurance benefits.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs such as initial assessments, follow-up consultations, and group support, but does not include unexpected costs that arise during treatment (i.e no show fee, etc.).

  • The Good Faith Estimate will be provided to you prior to your scheduled appointment, and must be ‘read and acknowledged’ at least 48 hours prior to seeing a practitioner at Enlighten Nutrition Counseling and Care.

  • A saved copy will be provided under your designated account upon beginning to receive care with our practitioners.

  • If you receive a bill that is a minimum of $400 more than your Good Faith Estimate, you can dispute the bill.

For questions or for more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 1-800-985-3059
